Key Findings:

  • Forty-four MFP states transitioned 11,440 new participants during 2015, which represents a 7 percent increase over the number of new participants transitioned during 2014 (10,658).
  • From January 2008 to December 2015, the cumulative number of individuals that ever transitioned to the community through MFP totaled 63,337, which represents a growth of 23 percent over the cumulative number transitioned as of December 2014 (51,676).
  • In the aggregate, grantees achieved 95 percent of the total transition goal for the year, having transitioned 11,440 new participants of the 11,985 planned for 2015.
  • The 44 MFP grantee states reported their total qualified home and community based services expenditures (total federal and state funds spent on LTSS for all Medicaid beneficiaries, including but not limited to MFP participants) for 2015 totaled approximately $74.5 billion, an increase of 5 percent from 2014 ($70.3 billion).
This report summarizes the progress of the Money Follows the Person (MFP) demonstration in the 44 grantee states (including the District of Columbia) that were actively transitioning MFP participants in 2015. The MFP program assists states with rebalancing their Medicaid long-term care systems and helps individuals living in institutions to move to community-based settings. This report presents key indicators of progress, including the number of new enrollees, states’ progress towards 2015 transition goals, long-term services and supports expenditure levels, rates of reinstitutionalization among MFP participants, types of qualified housing for new enrollees, and employment supports and services for MFP participants.
