This guide provides performance management strategies for practitioner agencies to measure the performance of initiatives to enroll people in Medicaid upon release from incarceration and connect them with health care services in the community. The guide details the steps for performance management and outlines potential performance metrics specific to Medicaid enrollment, prescription continuity, and connection with community-based care.
