| 摘要 | High and growing Medicaid costs have led to demands to control, limit, or cap program expenditures. This book identifies the sources of recent growth in Medicaid costs and evaluates proposals for changing the program. The analysis focuses on proposed incentives for states, providers, and recipients to alter patterns of service use and to control costs. Simulating the outcome of several financing proposals, the authors show why a joint federal-state program is preferable to financing solely by the states or solely by the federal government. They identify financing mechanisms that would lead to more equitable distribution of Medicaid benefits and the associated tax burden. They also consider direct controls on Medicaid costs: rate setting, HMOs, copayments, prudent buyer arrangements, health plans. They recommend changes to stimulate competition, including a system of vouchers. Read the full PDF. Thomas W. Pauly, professor of health care systems and public management at the University of Pennsylvania, associate editor of the Journal of Health Economics, and for the academic year 1982–83, research fellow at the International Institute of Management in Berlin, is the author of “Doctors and Their Workshops” and the editor of “National Health Insurance: What Now? What Later? What Never?” (AEI, 1980). |
